CONSULTATION

Your intial appointment will be with Dr. Pelletier to review your medical history, perform a through examination, and obtain and examine radiographs if necessary. After this he will explain your diagnosis and provide all treatment options. For some complex cases, study models, intra-oral photographs and other records may be obtained and a second consultaion visit may be needed.

Please assist us by providing the following information at this first visit::

A list of medications you are presently taking

If radiographs have been taken within the last year, please have a copy of them sent to our office before this first visit, or bring them with you.

If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

All patients under the age of 18 years of age must be accompanied by a parent or guardian at the first visit.

SCHEDULING

BASIC POLICY

Payment for services performed is due in full at the time of the service. Our office accepts cash, personal checks (there is a $35 returned check fee) and credit cards (Visa, Master Card, & Discover).We do offer financing plans.

FOR PATIENTS WITH INSURANCE

As a service to our patients, we will bill your insurance carrier, provided proper paperwork is provided to us. We will also assist you in billing your secondary insurance carrier, if applicable. Every effort will be made to estimate your portion and decuctibles that are due at the time of service, but the ultimate responsibility for any unpaid balance rests on you. Please understand that dental insurance is a contract between you, your employer and your insurance company. If an insurance company has not paid within 60 days of billing, any unpaid professional fees are due and payable in full from you.

NON-COVERED CHARGES

Any charges not paid by your insurance company will require payment in full at the time services are provided or upon notice of insurance claim denial. To assist our patients, we offer financial arrangements and/or alternative financing sources. Please ask our personal for additional information.

CANCELLATION OF APPOINTMENTS

Appointments with Dr. Pelletier and our staff have been reserved with a specific amount of time just for you.We do our very best to stay on schedule and on time for your appointments. Arriving late or canceling an appointment with less than 24 hours' notice disrupts this scheduling and adds to the cost of dental services. The practice reserves the right to dismiss patients with excessive late or canceled appointments.

EXPLANATION OF FEES

Your fee for service includes your visit with the doctor or hygienist based on the time, your dental condition, and any treatment provided .In addition, proper attention to your case may require time spent by the doctor outside of your direct visit with him. Such time may include:

  • Creation of a permanent dental record.
  • Review of current or previous radiographs
  • Mounting and use of study models, diagnostic wax-ups.
  • Preparation of letters, consultation reports.
  • Phone consultation with other physicians, or other dental specialists involved in your treatment.
  • Calls to and from pharmacies.
  • Processing and completing dental insurance forms.
